WHAT TO KNOW ABOUT AUDUBON PRESCHOOL’S HEALTH OFFICE

1. ATTENDANCE
If your child will be absent from school, please call the school or email the nurse/teacher to report his/her absence and the reason.


DOCTOR’S NOTE: A doctor’s note is required for 5 consecutive days of absences
related to illness before a student can return.


FYI: School procedure is to call when a student is unexpectedly absent to ensure
the safety of our students and families.


2. HEALTH PHYSICALS Universal Physical Form
In accordance with New Jersey State Law, all preschool students are required to present
documentation of having a physical examination at the beginning of each school year in
order to attend. Physicals must be completed within 365 days prior to beginning school.


3. IMMUNIZATIONS Preschool Immunization Requirements
If your child is not up-to-date with immunizations, you will receive a notice. All
immunizations need to be completed prior to the start of school (September 5th)

 

4. INFLUENZA (FLU) VACCINATIONS
New Jersey Immunization Regulations for School Attendance requires influenza for all
children between the ages of 6 months to 59 months attending day care or pre-kindergarten programs in New Jersey. Those children NOT receiving the influenza vaccine will NOT be allowed to attend school from January 1st until March 31st
.

5. MEDICAL CONCERNS/INJURIES
Medical: If there are any medical concerns for your child, please contact the school nurse.
We will work with you to help your child have a positive experience at school as well as
meeting his/her health needs.
Injuries: If your child has an injury and is unable to participate in recess or certain
activities, please contact the school nurse and send in a doctor’s note stating the restriction
and the length of time.


5. MEDICATION PROCEDURES:

If your child is diagnosed with a life threatening allergy and requires Benadryl and/or an
Epi-Pen, please have your child’s doctor fill out the Allergy Action Plan. Please return the
completed form, along with any prescribed medications to the health office.

If your child has a medical diagnosis of asthma, please have your child’s doctor fill out an
Asthma Action Plan. Please return the completed form, along with any prescribed
medications to the health office.


MEDICATIONS Medication Permission Form
If your child needs to take medication at school, whether it is prescription medication or non-prescription medication (over the counter), including cough drops, a medication permission form will need to be completed by both a physician and the parent.


6. FOOD
You will be notified of any allergy concerns for your classroom. Please be mindful of the
identified allergies in the classroom when sending in food with your child. This is to ensure
the safety of all of our students.
** All food for parties/events MUST be prepacked with a label. No homemade food will be allowed**

 

WHEN TO KEEP YOUR CHILD HOME

If your child has any of the following symptoms, keep him/her home, or make
appropriate child care arrangements.

● Fever: Temperature of 100.4 degrees Fahrenheit or higher. Child must not
have a fever for 24 hours without fever reducing medication before
returning to school.


● Vomiting: Child should not return to school for 24 hours following the last
episode of vomiting.


● Diarrhea: Children with watery stools may not return to school until 24
hours after their last episode of diarrhea.


● Eye Infection: Pink eye (conjunctivitis) or thick mucus or pus draining
from eye should not return to school for 24 hours after starting treatment.

OTHER HEALTH CONDITIONS

Impetigo: Children need to be excluded from school until at least a full 24 hours
after antibiotic treatment has begun and there is no discharge.


Lice: Parents/guardians will be notified by the school nurse who will determine if
the child needs to be sent home. Prompt treatment will need to be started prior to
return to school. See district policy for further information.


Chronic cough and/or runny nose: Continuous cough with greenish nose
discharge may be a sign of a contagious illness and may require treatment from your
healthcare provider.


Strep Throat: May not return to school until treated with antibiotics for a
minimum of 24 hours.


Rash: with any rash, especially with fever or itching, please contact the school nurse
prior to attending school for further guidance. Some rashes can be contagious.


Unusual appearance behavior: consider keeping your child home with the
following: Abnormally tired, pale, lack of appetite, difficult to wake, confused or
irritable.

5. Audubon School District Covid Guidelines
TESTED POSITIVE:

If your child tests positive for COVID-19, regardless of symptoms:
ISOLATE: Your child should isolate (separate for others) at home for at least five full
days from when they began experiencing symptoms and avoid being around others as
much as possible. Keep home from school, child care, and extra-curricular
activities/sports. Notify the school.
MASK: Have your child wear a well-fitting mask whenever around others through day
10.
MONITOR: Monitor your child’s symptoms and call their medical provider for
worsening illness or any symptoms that seem severe or concerning to you.
RETURN TO SCHOOL on Day 6 if:
       ○ They have been fever free without the use of fever-reducing medications; and
       ○ Other symptoms of COVID-19 are improving
       ○ A well-fitting mask can be worn through DAY 10.
NOTIFY the school and anyone they may have been in close contact with during the 2
days prior to the start of their symptoms through Day 5, or for those who did not develop
symptoms from the date the test was taken through Day 5.
