Please review the steps below carefully to ensure your student’s registration is completed smoothly and efficiently.Step 1: Complete Online Pre-Registration

Step 2: Gather Required Documentation
Please have all required documents ready before scheduling your registration appointment. You may use the list below as a checklist to help you prepare.
Student Records
Please bring the following items to your registration appointment:
- Completed registration forms (print and complete the appropriate grade packets for your student(s)
- Original birth certificate (a copy will be made during your appointment)
- Custody paperwork (if applicable; must indicate primary parent and/or state agency)
- Transfer card from former school
- A-45 health record from former school nurse
- Most recent report card
- Transcript and disciplinary record from former school
- Standardized test scores from former school
- IEP or 504 documentation (if applicable)
Note: If your student receives special education services or has an IEP, please notify the registrar
as soon as possible.
⚠️ Important Notice for Parents/Guardians
Please bring a valid photo ID (driver’s license or passport) to your appointment.
- The address on your ID must match the address listed on your primary proof of residency.
- If you have recently moved and your driver’s license does not yet reflect your new address, please complete and print a New Jersey Motor Vehicle Commission (NJMVC) Address Change Form and bring it with you.
Step 3: Provide Proof of Residency
Proof of residency must show the parent/guardian’s legal name.
If none of your documents list the parent/guardian’s name, a notarized Residency Affidavit
from both the district resident and the parent/guardian is required.
Both parties must attend the registration appointment in person.

Primary Document (choose one):
- Property mortgage bill (dated within 60 days)
- Property tax bill (dated within 60 days)
- Housing agreement (Audubon Park residents)
- Lease agreement (dated within the last 60 days)
- Must include the Audubon property address, names of both tenant and landlord, lease start/end dates, and names of all residents (if applicable).
- Lease must reflect a current, valid term consistent with the school year.

Additional InformationTo complete registration, the student must be accompanied by a parent or legal guardian,
unless one of the following applies:
- The student is not living with a parent or guardian and can establish proof of residence within the borough (must provide appropriate domicile papers).
- The student is 18 years of age or older.
- The student is an emancipated minor.
